How to Apply
About the Award
The P.L.A.Y. Award Fund was established at The Dallas Foundation to provide awards to amputee children and young adults (5-18 years of age) to enjoy music, arts, sports and sports-related activities and camps.
Activities:
The P.L.A.Y. Award Fund is designed to assist children and young adults to participate in camps or activities such as horseback riding, hockey, golf, skiing, running, camping, biking, etc. There are numerous camps and activity centers available throughout the country. Applicants can review some suggested available activity options on our Resources & Activities Page. These are suggestions and ideas to help you get started. However, P.L.A.Y. is for you, the applicant. We would like to hear about any activity that you would like to participate in. Make a suggestion in your application and we will do our best to make it a reality.
Who is Eligible:
You are eligible to apply for a P.L.A.Y. Award Fund if you or the child/teen for whom you are applying meet all of the following criteria:
- You are an amputee or have a limb absence from birth
- You are 5-18 years of age
- You have a prosthetic, if needed
- You are medically cleared to participate in the sport or activity chosen
- You are a resident of the United States
- You are unable financially to participate in the sport or activity without financial assistance
The Application Process - How to Begin P.L.A.Y.ing:
To apply for a P.L.A.Y. Award Fund a candidate must submit:
- Download, print,and complete an application form - click here to download.
(If you do not already have Adobe Reader on your computer and therefore cannot open the application, you can go to Adobe to download the free Adobe Reader by clicking here or you can email us at info@playfoundation.net to request an application be mailed to you.)
- Include with the submitted application form three letters of recommendation. These should help detail what the applicant hopes to accomplish with P.L.A.Y. and the chosen activity.
- One from a parent or guardian (and should include information about the need for financial assistance)
- One from a health care provider (physician, nurse, prosthetist, therapist, etc)
- One from someone who is not related to you
- Include with the submitted application form a description and proposed budget for the desired activity with as much detail as possible. A blank budget form is included with the application form to assist you.
- Include with the submitted application form a typed essay of no more than 500 words. This personal statement should be written by the child/teen if age allows for this (personally written is preferable), and should explain why you should receive the award and how the proposed activity will benefit you. Include any awards or recognition you have received for this activity in the past, if any, and reasons for choosing this particular activity. Funding is only available for one activity per candidate if award is granted.
Deadlines:
- Applications for activities must be received no later than 60 days prior to the requested event/activity

Only completed applications with all the above attachments will be considered. Any incomplete application will be returned. Faxed or emailed applications will not be accepted. Applications with attachments should be mailed to:
